RESERVATION POLICIES:

Please call to schedule an appointment. We are by appointment only. You can also use the “contact us” link, but please know the best way to reach us is to call 318-817-9900 for Carmel office or 765-564-1900 for Delphi office.

CANCELLATION POLICY:

We kindly ask you to let us know if you need to reschedule or cancel your appointment 24 hours or more prior to your scheduled visit. We reserve the right to charge a missed appointment fee if this 24 hour notice is not given.

CHECK-IN:

We ask that you arrive 15 minutes prior to your initial visit to allow time to check-in and complete necessary paperwork. Please bring insurance card if you choose to have us file with your insurance. You can also print paperwork here or by using the resources page and clicking “new patient forms”, download the pdf, print, and fill out to bring with you to your appointment.

FACILITIES:

We have two office locations for your convenience. Our Carmel, IN location on the north side of Indianapolis or our Delphi, IN location east of Lafeyette. Please ensure you let us know which you prefer when scheduling.

MEDICAL CONCERNS:

Please let us know any and all related health concerns on your entry paperwork and st your initial office visit so that we can come up with an appropriate treatment approach or in some cases referral.

Privacy Policy

Our privacy policies are included in the new patient paperwork and are available upon request.

Insurance

We are IN-network with most major medical plans but this is subject to change at any time. Currently we can file with Aetna, Anthem, Blue Cross Blue Shield (BCBS), Cigna, IU, Medicare, United Healthcare (UHC), and UMC. Please call your specific plan number on your card for more information on benefits and IN-network status.
